Apply for an Agency Card - - Borrow a Kit

To borrow a kit, Surrey groups will need an Agency Card. To apply, the group director/owner completes an Agency Card application and writes a letter on letterhead (or provides a current business licence or daycare registration confirmation from Child Care Options). Click here for Application Form.

Agency Cards are issued in the group's name and may be used by anyone within the group who is authorized by the groups' signing officer.  These cards are not intended for personal use.

The group's signing officer is responsible for materials borrowed on the card. Regular loan periods, fees and responsibilities apply (such as a loan limit of 35 items per card.)

Agency Cards expire annually, your organization would need to re-apply every year.
